Laporan Ketua Audit Negara 2009 sekali lagi menggemparkan. Tapi laporan Audit yang lalu pun banyak yang gempar pun kemudian senyap. Melayu Mudah Lupa. Kali ini kisah projek sakit Hospital Tentera dengan kos RM 268 Juta dan sandwich kosong tanpa sardin, tomato dan timun untuk murid sekolah.

SANDWICH tanpa sardin, tomato, timun dan salad diberi kepada murid adalah antara kelemahan dikesan Ketua Audit Negara dalam laporannya berhubung Rancangan Makanan Tambahan (RMT) di sekolah.

Laporan itu juga mendedahkan murid yang menyertai rancangan berkenaan turut diberikan mi segera dan bihun goreng kosong, serta tidak mematuhi menu ditetapkan Kementerian Pelajaran. Malah, makanan disediakan juga tawar atau masin, nasi keras, manakala minuman terlalu manis atau terlalu cair. “Antara kelemahan pelaksanaan RMT ialah kelayakan menerima tidak dipatuhi, makanan disediakan tidak berkualiti dan tidak mematuhi panduan, pengurusan dan penyediaan makanan kurang memuaskan, serta kelemahan dalam pemantauan,” kata laporan itu.

Seramai 689,950 murid di 138 daerah di seluruh negara menerima bantuan RMT membabitkan perbelanjaan berjumlah RM249.26 juta pada 2009.

“Pemeriksaan audit juga mendapati ada makanan seperti kari daging lembu disimpan selama 18 jam, iaitu dimasak pada 16 Julai 2009 sebelum jam 1 tengah hari untuk dihidangkan kepada murid pada keesokannya jam 7.30 pagi,” kata Laporan Ketua Audit Negara 2009.

Di Sekolah Kebangsaan (SK) Bokah, Lundu, Sarawak, ibu bapa terpaksa bergilir masak di stor yang tidak sesuai, berhabuk, kotor serta terbuka, manakala murid pula makan di kelas.

Selain itu, kantin sedia ada di SK Pagar Besi, Kuala Terengganu, dirobohkan untuk membina bangunan baru bilik darjah, makanan terpaksa dimasak di kuarters dan murid makan di tempat letak kereta yang terdedah kepada habuk serta keadaan berbecak apabila hujan.

RM268mil armed forces hospital is ‘sick’

MANY things are ailing the RM268mil armed forces (AF) hospital in Kuala Lumpur – from the reduction of its planned size without approval from the Finance Ministry (MOF), poor design and freezer rooms that did not work.

According to the Auditor-General’s 2009 Report, the hospital – called “95 Hospital ATM” – which began operations in 2009 was not fully operational because of poor construction work and a delay in securing the allocation for linen.

It said the hospital had 60% less medical equipment from the initial plan, was without air-conditioning along the corridors, had an intercom system implemented in 15 critical stations instead of the proposed 52 stations, and had lost 7,000sq m of built-up area from the original plan.

Perbadanan Perwira Harta Malaysia (PPHM) was awarded the RM267.89mil design-and-build contract through direct negotiations, while QSC Projects Sdn Bhd (QSCP) was appointed the project management consultant.

Although the contractor had asked for a revision on the scope of development, the report said there was no proof that MOF had approved the amendments.

In its reply, the Defence Ministry (Mindef) said that QSCP had failed to notify MOF that the initial cost projection was inaccurate and that the scope of work would have to be reduced if the price of RM267.89mil was maintained.

“MOF should have been informed that the estimated cost of RM267mil quoted by the project management consultant cannot be based on the contractor’s pre-bid documents during the price negotiations.”

Mindef added that QSCP’s “unprofessional” services had been terminated on Aug 19, 2004. Mindef said it had appealed to MOF to consider a new price of RM341mil, which had been agreed to by it and PPHM.

“However, on July 19, 2002, MOF insisted on maintaining the price of RM267.89mil, saying that if the negotiation price could not be agreed upon, the project would be re-tendered,” said Mindef.

Since the contractor was unable to implement the full scope of the project, Mindef said a reduction was agreed upon to complete it.

The report also noted that although the hospital started operations on Sept 28, 2009, only the specialist clinics, primary care department and dental department were operating as on Dec 10, 2009.

According to the report, the project was marred by poor design – there were unsuitable baby bathtubs in the paediatric and maternity wards and a wall separated the nurses’ station and patients at the haemodialysis unit.

Also, the cold and freezer rooms in the medical and dental depot were not working, the hospital was using old refrigerators to store medicine and the wards and operating theatre could not be utilised because the allocation for linen had not been applied for.
